It happened on Monday night on Clinton Avenue in the village of New Square.
Authorities said it appears the driver fell asleep at the wheel, setting off a series of collisions.
The accident happened at around 3:15 p.m. at Einstein Loop north and Hutchinson River Parkway east in Co-Op City.
