The breach occurred between Feb. 19 and Feb. 25, and UnitedHealthcare determined personal information may have been affected on April 10.
CBS 2 has obtained a letter sent to employees by Jewel-Osco parent company Albertsons last week.
As of Monday night, it was not clear how many employees had been impacted.
Advocate Aurora Health said, based on their investigation, no patients' Social Security numbers, financial accounts, or credit or debit card information were involved in the breach.
