Final Report From Chemical Safety Board On West Fertilizer Explosion

WEST (CBSDFW.COM) - It's been nearly three years, but the U.S. Chemical Safety Board has released its final report on West Fertilizer Company plant explosion.
The report goes into detail about some information that was already known – like the lack of oversight of the ammonium nitrate at the plant and limited to non-existent planning or training for employees.
The report lists three scenarios for how the explosion happened, and says there are 19 other places in Texas where a risk to the public still exists.
In figuring out what happened, the report puts a lot of the focus on the way ammonium nitrate was stored. The heat, pressure and debris from the fire, could have all contributed to causing the explosion.
The report found fault at all levels and includes recommendations to nearly a dozen different agencies. Most of those recommendations focus on new standards and training for handling ammonium nitrate.

Officials dedicated the report to the 15 people who died in the April 17, 2013 explosion. The blast also injured more than 250 others.
The Chemical Safety Board is meeting in Waco on Thursday to hear a presentation on the report, and answer questions from the public.
