NEW YORK (CBS) Kate Major, the fiancee of Michael Lohan, the father of jailed actress Lindsay Lohan, claimed that Michael threatened to kill her and called her a "c**t," right before he kicked her in the face, according to a police report and official complaint obtained by TMZ.

Kate Major reportedly filed the complaint with the Southampton Town police; in it she details a series of events that she says occurred July 19 at approximately 2:00 p.m.

The complaint says that Major was sleeping in a sofa chair when Lohan woke her, yelling, "Why didn't you pick up your cell phone you stupid c**t."

According to the complaint published on TMZ, Lohan then turned over the chair Major was in, tossing her to the floor. "I crawled to the other side of the room to get away from him," it reads. "He walked over to me, took his shoe off and kicked me in my face while I was still on the ground. He stood over me and said, "I'm going to go back to jail cause of you, cause I will kill you."

Major also indicated in the report that Lohan allegedly abused her because of his fear that she will leave him. "I am very afraid of him," it reads.

Michael Lohan said the two argued, but he denied ever laying a hand on Major, reports TMZ

He is slated to appear in court August 18 to be arraigned on harassment charges

Prior to her relationship with Lohan, Major, a former Star magazine reporter, claimed she had a "steamy" 12-day affair with reality TV personality Jon Gosselin of the TLC show "Jon & Kate Plus 8."
