(CBS) BOSTON - Police have issued an Amber Alert for a Westfield woman who has taken her three-month-old daughter, of whom she does not have custody, reports CBS Boston.

The girl may be in extreme danger, according to investigators.

Authorities say Cheryl Kozaczek, 28, left the family's home in Westfield at 1 a.m. Wednesday with her three-month-old daughter Gaebria in a gray 2004 Toyota Tacoma pick up truck with Massachusetts plate 85ZH15.

Kozaczek's mother is the child's legal guardian.

Police have distributed a photo of what that model of truck looks like. Kozaczek's vehicle has a Westfield State University parking decal on it.

Cheryl Kozaczek is described as white, 5-feet 4-inches tall, weighing about 115 pounds. She has long brown hair with blond highlights, often worn in a ponytail. She was last seen wearing a lightweight tan sport coat, black jeans, and black Army boots with yellow stitches.

She may be carrying a large black diaper bag.

Gaebria is white, about two feet tall and weighs approximately 12 pounds. She has a full head of brown hair and brown eyes. She was last seen wearing pink and black striped tights with either what is a described as a fluffy top or a pink night shirt.

State police say Kozaczek has previously spent time in California and will sleep in her pickup truck or in hotels.

The Boston Herald reports that anyone who sees her, the daughter, or her truck is urged to call state police at 508-820-2121.
