ULTRA FOURSOME GOLF GIVEAWAY CONTEST OFFICIAL RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1. HOW TO ENTER
a. The Ultra Foursome Golf Giveaway Contest (the "Contest") begins on July 10, 2012 at 7:00 AM Eastern Time ("ET") and ends on August 3, 2012 at 9:00 PM ET (the "Entry Period"). The Contest shall be administered by WXYT-FM, 31555 W. 14 Mile Road, Ste. 102, Farmington Hills, MI 48334 (the "Station").
b. To enter the Contest by text message, listen to the Station during the Entry Period for the Station's on-air talent to announce the special text code ("Short Code") and text the Short Code to 97136 during the Entry Period.
Message and data rates may apply. The Station is not responsible for any fees incurred by an entrant for this method of entry. Potential prize winners in a text message promotion must provide all information requested including their full name, complete address (including zip code), day and evening phone numbers and date of birth to verify eligibility. By entering by text, you consent to receive a confirmation text message from the Station.
c. To enter the Contest on-site, visit one (1) of the following four (4) Station appearances ("Station Appearances") at the dates and times listed to enter:

At a Station Appearance complete an official entry form (while supplies last) including first name and last name, complete address, city, state, zip code, home telephone number and date of birth, and put the entry form in the registration box. All entries must be received by the end time of each Station Appearance listed above. The Station is not responsible for lost, stolen, mangled, misdirected, postage due, illegible, incomplete or late entries.
d. Only one (1) entry per person is permitted regardless of the method of entry.
2. ELIGIBILITY RESTRICTIONS
a. The Contest is open to all residents of the state of Michigan who possess a valid government-issued picture ID, and are 21 years of age or older at the time of entry. Employees of the Anheuser- Busch, and CBS Radio Inc. (collectively, "Contest Entities"), and each of their respective parent, subsidiary and affiliated entities, Station's advertising and promotional agencies and participating sponsors, other radio stations in the Detroit Arbitron Metro, the members of their immediate families (spouse, parent, sibling or child) and those living in the same household of each (whether related or not), are ineligible to enter or win. This Contest is subject to all applicable federal, state and local laws and regulations and is void where prohibited. Entrants are responsible for complying with these Official Rules.
b. Winners must meet all eligibility requirements to receive any prize package.
c. The winner must provide valid government-issued photo identification and provide their complete address, date of birth and phone number to claim a prize.
d. Entrants are required to provide truthful information and the Station will reject and delete any entry that it discovers to be false or fraudulent. The Station will disqualify any entry from individuals who do not meet the eligibility requirements and will also delete any entry as required by law.
3. PRIZES
a. One (1) Grand Prize will be awarded. The Grand Prize is a trip to Jacksonville, Florida for four (winner and three guests 21+). The Grand Prize includes two (2)-nights hotel accommodations (two rooms, standard double-occupancy at a hotel of Station's choice), a round of golf, carts and lodging at the TPC Sawgrass in Jacksonville Florida, and round-trip coach airfare for winner and three (3) guests from Detroit to Jacksonville, Florida departing on August 29th, 2012 and returning on August 31st, 2012 (Date subject to change). Grand Prize Approximate Retail Value ("ARV"): $3,700*. (But actual value of each Grand Prize may vary depending upon cost of airfare at time of airfare purchase.)
b. Certain travel restrictions may apply. All travel arrangements will be made by Station in their sole discretion and certain restrictions may apply. All other costs not specifically stated herein, including but not limited to those of, taxes, meals, tips and ground transportation to/from the airport and/or to/from the hotel, are the responsibility of the winner. Each Grand Prize winner and their travel companion must travel on the same itinerary and are responsible for obtaining all required travel documents. Each winner and travel companion must sign a Publicity, Liability Waiver and Travel Release form prior to traveling. . If applicable, the winner may be required to present a valid credit card upon hotel check in to cover any incidental expenses incurred during the winner's stay. If a Grand Prize winner is unable to travel on the date as determined by Station, such prize winner will forfeit the prize package in its entirety and an alternate prize winner may be selected from all remaining eligible entries. Station and Sponsors are not responsible for flight delays or cancellations, game time changes or cancellations. GROUND TRANSPORTATION WILL NOT BE PROVIDED AS PART OF THE GRAND PRIZE PACKAGES. Other travel restrictions may apply. If a selected event is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, or for any reason beyond the control of the Station, the Station's sole responsibility to the winner will be to award the remaining available portion of the prize and no substitution or compensation will be provided for the unawarded portion of the prize. The Station reserves the right to remove or to deny entry to the winner and/or travel companion(s) if either engages in a disruptive manner or with intent to annoy, abuse, threaten or harass any other person at any Grand Prize Prize-related event.
c. One (1) Second Prize will be awarded. The Second Prize is an opportunity for winner and one (1) guest to play eighteen holes golf with the Station's Doug Karsch and Scott Anderson at Brae Burn Golf course in Plymouth on a date to be mutuality agreed on by the station and talent by the Station in consultation with the Second Prize winner. Second Prize Approximate Retail Value: $54.00 (Price Subject to change)
d. All other costs not specifically stated herein, including but not limited to those of, taxes, tips and ground transportation to/from the golf course, are the responsibility of the winner. The winner and guest must sign a Publicity and Liability Waiver Release form prior to participating in any Second Prize-related event. If a selected event is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, or for any reason beyond the control of the Station, the Station's sole responsibility to the winner will be to award the remaining available portion of the prize and no substitution or compensation will be provided for the unawarded portion of the prize. The Station reserves the right to remove or to deny entry to the winner and/or guest(s) if either engages in a disruptive manner or with intent to annoy, abuse, threaten or harass any other person at the golf course.
a. Prize certificates must be picked up at the office of the Station at the address listed below. Prize winners will forfeit any prize or prize certificate not claimed by August 10, 2012 of winning and an alternate winner may be selected in a random drawing from all remaining eligible entries (time permitting). Prizes not claimed within the stated time limit will be forfeited. Forfeited prizes become the property of the Station.
b. There are no substitutions, transfers, or cash equivalents for prizes, except that Station may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
4. SELECTION OF WINNERS
a. Decisions of the Station management with respect to the Contest are final.
b. On August 6, 2012 at approximately 9:00 a.m. the Station will select one (1) potential Grand Prize winner and one (1) potential Second Prize winner through a random drawing from all valid text entries and on-site entries received during the Entry Period.
c. The Station will notify the Grand Prize winner and the Second prize winner on August 6th, 2012 following the Grand Prize and Second Prize winner drawing. The Grand Prize and Second Prize will be awarded upon confirmation of eligibility.
d. Entrants need not listen to the Station nor be present to win the Grand Prize and/or Second Prize.
e. Grand/Second Prize winners must execute and return any required affidavit of eligibility and/or liability/publicity release by August 10, 2012 or prize will be forfeited and Stations may elect not to award the prize to an alternate winner.. Prize(s) will be awarded only upon confirmation of eligibility and completion of all requisite releases. If a potential contestant or winner cannot be contacted, fails to sign and return the required fully-executed affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, the potential winner forfeits the prize.
5. CONDITIONS
a. Payments of all federal, state and local taxes are solely the responsibility of each winner. Each winner will be required to complete and submit an IRS Form W-9 with the winner's full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station (or any other CBS Radio station in Station's market) in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize. Such winnings of $600 or more will be reported to the IRS.
b. By participating in the Contest, each winner agrees to have the winner's name, voice and likeness used in any advertising or broadcasting material relating to the Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require Contest winners (and any travel companion(s) or guest(s)) to sign a liability release, agreeing to release and hold harmless the Sponsors, the Station, its licensee, their respective parent, subsidiary and affiliated entities, their respective officers, shareholders, directors, employees, agents and representatives and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the prize and participation in the Contest.
d. If for any reason this Contest cannot be executed as planned, including, but not limited to, as a result of infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Station that corrupt or affect the security, administration, fairness, integrity or proper conduct of the Contest, or if the Contest is compromised or becomes technically corrupted in any way, electronically or otherwise, the Station reserves the right to cancel, terminate, suspend, or modify the Contest. If the Contest is terminated before the original end date, the Station will select the winners by random drawing from among all eligible non-suspect entries received as of the termination time/date.
e. The Station, in its sole discretion, reserves the right to disqualify any person tampering with the entry process or the operation of the Station's website. Failure to comply with the rules of the Contest may result in a contestant's disqualification and/or forfeiture of any prize or prizes. If the Station makes a good faith determination that an entrant has cheated or committed fraudulent activity in connection with a Contest, the Station reserves the right to disqualify that entrant from entering and/or winning future Contests and to prosecute and seek damages to the fullest extent permitted by law.
f. The Station reserves the right to make changes in the rules of the Contest, including, without limitation, the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Sponsors or Station, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel, terminate, suspend, or modify the Contest and shall not be required to award a substitute prize.
g. The Sponsors and the Station are not responsible for typographical or other errors in the printing, the offering or the administration of the Contest or in the announcement of a prize.
h. For the Official Rules, send a self-addressed stamped envelope for receipt by August 3, 2012 to "Ultra Foursome Golf Giveaway Contest – Official Rules" c/o 97.1 The Ticket, 31555 W. 14 Mile Road, Ste. 102, Farmington Hills, MI 48334. For the names of the prize winners send a self-addressed stamped envelope for receipt by October 3, 2012 to "Ultra Foursome Golf Giveaway Contest – Winner List" at the above address. Official Rules and Winner Lists (when completed) shall also be available during regular business hours at the main offices of the Station.
