College Prep Should Include 3 Important Forms
DETROIT (WWJ) - Add three things to your kids' "back to college" list: a "Durable Power of Attorney", a "Medical Authorization Form" and a "Designation of Patient Advocate."
Attorney Joe Bonventre, from Clark Hill, says if the un-thinkable happens and your child is injured or gets sick on campus, you're going to need them.
"Under the HIPPA privacy rules, a hospital, a doctor, a nurse -- they could be fined thousands of dollars if they disclose personal health information, which would include information about a son or daughter to someone else who doesn't have the right authority," Bonventre said.
