NTSB blames D.C. midair collision on systemic failures: "100% preventable"

Nearly one year after a Blackhawk helicopter and an American Airlines flight collided near Washington's Reagan National Airport, the NTSB blamed the accident on systemic failures within air traffic control, Army aviation and the FAA.
