AUSTIN (CBSDFW.COM) - The Texas Department of Aging and Disability Services (DADS) accidentally released protected health information for approximately 6,600 Medicaid recipients.

DADS is in the process of notifying them.

On April 21, DAD was informed the information was accessible on line on a DADS web application meant for internal use only.  DADS says it immediately took down the website and launched an investigation which is ongoing.

The information disclosed includes names, addresses, dates of birth, Social Security and Medicaid numbers, along with medical diagnoses or treatment information.  DADS says it has no reason to believe any of the information has been misused.

DADS has partnered with CSID to provide one year of free CSID protector services, which include credit monitoring and identity theft restoration.
