WACO (CBSDFW.COM) -  The U.S. Chemical Safety Board (CSB) - an independent federal agency charged with investigating serious chemical accidents, will present its final investigation report into the April 17, 2013, fatal explosion and fire at the West fertilizer facility at a January 28, 2016, news conference at 10:00 a.m. and public meeting at 6:00 p.m.

Both events will happen at  the Hilton Waco Brazos A&B Meeting Room.

The blast killed 15 people, destroyed homes and some of the city's schools, forcing students to start over in a different building out of town.

In April 2014, federal investigators called the fertilizer plant explosion in West preventable and said it should never have happened.
