Philadelphia officer identified in deadly Kensington shooting

PHILADELPHIA (CBS) -- Philadelphia police have named one of the officers involved in the shooting that killed a 27-year-old man in Kensington Thursday. This comes after police revealed new information, changing the narrative of what happened Monday.

More than 72 hours after the fatal incident, Philadelphia police are naming the officer as Mark Dial, who's been on the force for five years.

Meanwhile, the Citizens Police Oversight Commission say they're actively monitoring the investigation.

It was Monday afternoon when 27-year-old Eddie Irizarry was killed.

Originally, Philadelphia police reported Irizarry was outside his car and lunged towards officers with a knife. That narrative then changed.

On Wednesday, police said once body camera footage was reviewed, it showed Irizarry was inside the car when he was shot.

"I understand and want to acknowledge the hurt and confusion," Police Commissioner Danielle Outlaw said.

"It's never good to get it wrong, and we'll examine after the fact sort of how that happened to try to make sure those kinds of errors don't happen again in the future, but they came out and corrected it, and that speaks well of them," Anthony Erace said.

Erace is the Interim Executive Director of the Citizens Police Oversight Commission also known as CPOC.

The commission is independently monitoring the investigation.

"We have scene access, evidence access and we're an integral part of the process," Erace said.

Hours later on Wednesday, the family of the 27-year-old held a balloon release. They say their trust in police is in question.

"I just want everyone to tell the truth and even come here and come apologize to us," aunt Maria Irizarry said.

Erace: We all do. I think we all deserve that. And I think that part of CPOCs mission is to ensure that that happens.

Once the investigation is complete, Erace says the commission will provide two after-action policy reports, one on the incident and the other on the original narrative.

There's no timeframe for when those may be released.

"It's not going to be one of those things where when this is all said and done, people will have to wonder what really happened, right? Erace said. "That's one of the primary purposes of oversight."

There was a second officer on the scene when the shooting happened and police have said that officer did not fire their weapon.

At this time, police have not released that officer's identity.
