Suffolk County Police told CBS2 Sabrina Andrade, 28, was driving westbound on Connetquot Avenue when she crossed into the eastbound lanes and crashed head-on into a 2015 Ford SUV Saturday afternoon. Police said Andrade then fled the scene in her car and drove through a guardrail on the north side of Connetquot Avenue and hit a tree.

Andrade was taken to Southside Hospital in Bay Shore for a minor laceration to her forehead and released.

There were no other injuries reported by anyone else involved in either crash.

Andrade had children aged 3, 7 and 9 in her car at the time of the accidents.

She was arrested and charged with aggravated driving while intoxicated with a child passenger 15 years old or younger, driving while intoxicated, and leaving the scene of an incident, among others.

She is scheduled to be arraigned Sunday at First District Court in Central Islip.

The children were released to family members.
