Man Struck, Killed By Van In West Garfield Park

CHICAGO (STMW) -- A 38-year-old man driving a van on a suspended license struck and killed a pedestrian Tuesday night in the West Garfield Park neighborhood.

The man was driving a 2000 Dodge van northbound on Pulaski Road about 8 p.m. when the vehicle hit the pedestrian crossing the street eastbound at the intersection with Van Buren Street, police said.

The pedestrian, later identified as 33-year-old Cayode Fornizy, was not in the crosswalk at the time of the incident, according to police and the Cook County Medical Examiner's office.

Fornizy, of the of 2500 block of East 81st Street, was taken to Mount Sinai Hospital, where he was pronounced dead at 8:56 p.m., the medical examiner's office said. An autopsy is scheduled for Thursday.

The van's driver was cited for failure to yield to a pedestrian, driving on a suspended license and driving without insurance, police said.

The police Major Accidents Investigation Unit is investigating the crash.
