Department of Justice investigator Danny Santiago testified Thursday at a preliminary hearing about an interview he had with the nanny in the Bahamas in September.
Santiago says Nadine Alexie saw Smith's boyfriend Howard K. Stern and Dr. Khristine Eroshevich force Smith to take drugs that kept her asleep for days.
Santiago says the nanny claimed Smith would wake up with feces and vomit in her bed.
The hearing will determine whether Stern, Eroshevich and Dr. Sandeep Kapoor stand trial of charges of illegally funneling drugs to Smith.
Smith died of an accidental drug overdose in 2007.
