It is called a skin sparing mastectomy. Expanders are inserted and used to expand the muscle over time. Then implants are placed under the muscle in a later surgery. The patient's own tissue can also be used to create a new breast. This is a very involved surgery that takes many hours and a long recovery. Hope this answers your questions.
