Gluten-free diet fad: Are celiac disease rates actually rising?

(CBS/AP) Nearly two million Americans have celiac disease and should avoid eating gluten, a new study finds. However, as little as a decade ago, virtually no one in the U.S. seemed to have a problem eating the protein that's found in bread and other foods.

Does the national uptick in people eating gluten-free foods suggest a celiac disease epidemic or are people unnecessary turning to the diets as a food fad? According to a new study from the Mayo Clinic, it may be both. It found most people with celiac disease might not be aware they have the condition, and many people eating gluten-free diets have never been diagnosed with celiac disease.

For the study, Dr. Joseph Murray, a gastroenterologist at the Mayo Clinic in Rochester, Minn., and his team looked at blood samples taken from Americans in the 1950s and compared them with samples taken from people today and determined it wasn't just better diagnosis driving up the numbers. Celiac disease actually was increasing. Indeed, the research confirms estimates that about 1 percent of U.S. adults have it today, making it four times more common now than it was 50 years ago.

That translates to about 1.8 million Americans with celiac disease, but about 1.4 million people with the condition may not be aware they even have it. On the flip side, about 1.6 million people in the U.S. are on a gluten-free diet even though they haven't been diagnosed with celiac disease.

Celiac disease is a condition that damages the lining of the small intestine when a person eats gluten found in wheat, barley, rye or some oats, preventing the intestine from absorbing necessary nutrients. A strict gluten-free diet is often prescribed for people with the disease. Fads aside, the new study suggests more people are truly getting sick from the gluten found in wheat, rye and barley, but the reasons aren't clear. In the most serious cases, gluten triggers celiac disease, which can cause symptoms including abdominal pain, bloating and intermittent diarrhea. The lack of nutrients can cause sufferers to experience weight loss, fatigue, rashes and other problems. Celiac disease is different from an allergy to wheat, which affects a much smaller number of people, mostly children who outgrow it.

It was once considered extremely rare in the U.S. But about 20 years ago, a few scientists began exploring why celiac disease was less common here than in Europe and other countries. They concluded that it wasn't less common here; it was just under-diagnosed.

Scientists suggest that there may be more celiac disease today because people eat more processed wheat products like pastas and baked goods than in decades past, and those items use types of wheat that have a higher gluten content. Gluten helps dough rise and gives baked goods structure and texture.

Or it could be due to changes made to wheat, Murray said.

In the 1950s, scientists began cross-breeding wheat to make hardier, shorter and better-growing plants. It was the basis of the Green Revolution that boosted wheat harvests worldwide. Norman Borlaug, the U.S. plant scientist behind many of the innovations, won the Nobel Peace Prize for his work. But the gluten in wheat may have somehow become even more troublesome for many people, Murray said. That also may have contributed to what is now called "gluten sensitivity."

Doctors recently developed an ambiguous definition for gluten sensitivity. It's a label for people who suffer bloating and other celiac symptoms and seem to be helped by avoiding gluten, but don't actually have celiac disease. Celiac disease is diagnosed with blood testing, genetic testing, or biopsies of the small intestine.

The case for gluten sensitivity was bolstered last year by a very small but often-cited Australian study. Volunteers who had symptoms were put on a gluten-free diet or a regular diet for six weeks, and they weren't told which one. Those who didn't eat gluten had fewer problems with bloating, tiredness and irregular bowel movements.

Clearly, "there are patients who are gluten-sensitive," said Dr. Sheila Crowe, a San Diego-based physician on the board of the American Gastroenterological Association.

What is hotly debated is how many people have the problem, she added. It's impossible to know "because the definition is nebulous," she said.

One of the most widely cited estimates comes from Dr. Alessio Fasano, a University of Maryland researcher who led studies that changed the understanding of how common celiac disease is in the U.S. Fasano believes 6 percent of U.S. adults have gluten sensitivity. But that's based on a review of patients at his clinic - hardly a representative sample of the general public.Other estimates vary widely, he said.

"There's a tremendous amount of confusion out there," Fasano said.

Americans will spend an estimated $7 billion this year on foods labeled gluten-free, according to the market research firm Mintel. But the best estimates are that more than half the consumers buying these products - perhaps way more than half - don't have any clear-cut reaction to gluten. They buy gluten-free because they think it will help them lose weight, because they seem to feel better or because they mistakenly believe they are sensitive to gluten.

"We have a lot of self-diagnosing going on out there," said Melissa Abbott, who tracks the gluten-free market for the Hartman Group, a Seattle-area market research organization.

But, gluten-free diets may not lead to weight loss. In fact, for celiac patients weight gain for those on the gluten-free diets is the goal because it suggests they're absorbing the necessary foods and nutrients they had been lacking.

Whatever the reason, marketing of foods without gluten has exploded. Those with celiac disease, of course, are grateful. Until only a few years ago, it was difficult to find grocery and dining options.

"It's a matter of keeping people safe," said Michelle Kelly, an Atlanta-area woman who started a gluten-free, dairy-free, soy-free, nut-free bakery in 2010 after her son was diagnosed with celiac disease. While conventional bakers use wheat flour, she uses such ingredients as millet flour, sorghum flour, brown rice flour and tapioca starch.

At one of Atlanta's largest and busiest health food stores, Return to Eden, manager Troy DeGroff said over a third of his customers come in for gluten-free products for themselves or their family.

"Thank you, Elisabeth Hasselbeck," he said, referring to one of the hosts of the daytime talk show "The View" who helped popularize gluten-free eating.

It's hard to say how many of his customers have a medical reason for skipping gluten. But he said, "they're at least paying attention to what they're sticking in their mouth."

The study is published in the July 31 issue of the American Journal of Gastroenterology.

I've been a diagnosed Celiac since 1997 so yes there were gluten-free movements 17 years ago. It often takes extremely powerful grassrooot movements, growing education, research and ambassadors to get to the place we are today, which is only scratching the surface of awareness and product offerings.

The average gluten-free follower is not educated :( It's poorly understood that gluten-free means dieting and losing weight. That's a double edge sword. Gluten-free foods are all natural, non gluten containing grains which could result in losing weight and it's important to know that anemia or other health concerns may exist if you're not educated on overall foods nutrition which our schooling doesn't provide for everyone.

In addition the consumer packaged goods market is growing and offering a diverse health offering that ranges in gluten-free foods that may be too high in starch, sugars, calories to over compensate lack of taste due to the idea that American needs that taste and belly full experience.

Modern wheat was genetically modified in the 1970's by Norman Borlaug, a plant geneticist who won the Nobel Prize and Congressional Gold Medal for developing a high yield dwarf wheat which was a blessing because it saved two or three billion people from dying of starvation. The "curse" is the magnified adverse health effects. Changes to the gliadin protein in the new wheat have quadrupled the incidence of celiac disease and auto-immune disease. Opiate activity of semi-digested peptide fragments from the gliadin protein stimulate the appetite causing increased caloric intake and obesity. Amylopectin component causes hyperglycemia and insulin resistance diabetes, now reaching epidemic proportions in our population. For genetically susceptible people with celiac or autoimmune disease, eliminating wheat from the diet is curative. Your mainstream rheumatologist, gasteroenterologist or endocrinologist will deny or reject this connection, even though it is now published in mainstream medical journals.

A lot of people are switching to glouton free foods because it's the only answer they can find online for their bloating issue. What many people are having issues with nowadays is waking up with a flat stomach and then looking 7 months pregnant by the end of the day. I have this issue and my doctors have no idea what it is. I tested negative for one Celiac test but I don't think this is a glouton issue. I'd like to know why my stomach bloats up but I don't pass gas.

Actually I think the 1950's study has one really glaring flaw. They tested *pilots*. Pilots, by definition at that time, have to be healthy. Perfect eyesight even. Celiacs are generally *not* healthy people; they are the ones that are chronically ill with something, and often have IBS, which makes being a pilot difficult!

Also, in 1950, anyone who was a pilot, had to make it to adulthood without antibiotics. Celiac kids get infections a lot, and likely, a huge chunk of them never grew up.

Those pilots were mostly white males too, from a fairly narrow stock of genes. The genes for celiac vary depending on the group.

And last, babies born in 1940 or so, were mostly breast-fed. Infant formula makes for a very different population of gut bacteria, compared to breast-feeding.

Anyway, there are too many variables to say that "celiac is becoming more prevalent" based on one study. Modern wheat and wheat-products are different, for sure, but celiac is largely determined by genes, and the genes haven't changed much lately.

The moniker "healthy grains" is an oxymoron created by the food industry, big pharma and Corporate farming -- many people are now cutting out flour, processed foods and sugars, not only for maintaining a healthy weight, but for improving all health markers and turning back the age clock. Read the books "wheat belly" and the "the diabetes solution" or watch "fat head" the movie. Today at lunch I enjoyed a 16 oz rib-eye steak with a stick of butter, garlic and hot peppers. I haven't been very hungry all day. No need to count calories anymore. When I ate carbage while on SAD, I had to snack every few hours, count calories, and exercise my tail off or gain weight!

Not too long ago, I remember red meat being villified the way wheat/grains are now.

I have some wheat/gluten sensitivity, it comes and goes, but there are times that it is significant... a steady stream of kefir in my diet reduces the sensitivity, but isn't a permanent solution.

One of my children was super sensitive to gluten, getting better now at age 10, but around 6 and 7 it was scary what it did to him.

I wish more people would take your advice. The easiest and most consistent diet is to simply limit yourself to only unprocessed natural/organic foods. Then one does not need to count calories. Most illnesses today are diet-related and are due to industrial/processed foods which contain artificial preservatives, flavors, colors, trans fats, high fructose corn syrup, etc.

Actually, I've had digestive issues for as long as I've been alive, and the medical community did every test possible and could never diagnose the problem. Blood test for celiac disease never showed anything. I found a doctor who recognized the symptoms. In fact, I don't have celiac disease. From a biopsy, I was diagnosed with severe gluten intolerance. Now, I've been gluten free for 2 years, and I no longer have any digestive issues whatsoever!
AND from what I've been reading, manufacturers have added gluten protein to more products over the last 40-50 yrs and have probably increased the amounts of gluten to be just UNDER what is acceptable by the FDA. Maybe, the author of this article should have spent more time speaking to specialists at the Center for Celiac Disease in San Diego who would have informed him/her that celiac disease and gluten intolerance have many of the same symptoms. AND a biopsy of the small intestines is really the only definitive way to confirm the diagnosis.
For many of us who are gluten intolerant, it is imperative that we remain gluten free to maintain our quality of life. AND NO!, Replacing basic good nutritional food with over the counter prepackaged, highly preserved gluten free products is NOT a good idea either!

So right! I just bought some "gluten-free' cereal, and guess what?? One of the main ingredients was SUGAR. We must read labels

Years ago, hardly anybody heard of gluten free. Fad or not, gluten-free is here to stay for those of us with celiac disease. Social media could also be contributed to higher rates simply because of knowledge share. Navigating away from gluten isn't as hard as it used to be. The Annual Gluten-Free Awards highlight the best products services and organizations. "It's a people choice award for all things gluten-free" Jayme Schieffer, co-founder of GFREEK.com. The Annual Gluten-Free Awards are going into their 3rd year with voting beginning this Fall 2012. The newly diagnosed can feel confident with higher priced purchases knowing the gluten-free community is backing the product or service. This type of program didn't exist until 2010. Things are starting to look up.
