The show stars Laura Linney as Cathy, a woman diagnosed with cancer, who is trying to make the most of the time that she has left.
Saradon is on board to play Joy, a cancer survivor who uses her own personal experience to inspire others, reports The Hollywood Reporter.
The show has had plenty of big-name guest stars including Idris Elba, who played a painter with whom Cathy had an affair and Hugh Dancy, who played Lee a friend of Cathy's and fellow patient, who died last season.
This season, in addition to Sarandon, Victor Garber will also be guest starring, reports THR.
The third season of "The Big C" will premiere April 8 on Showtime.
