Health care power of attorney, which may go by a different name in your state, allows another person to make medical decisions on your behalf if you are not able to do so. This can be applied in short-term situations as well as terminal cases.

The American Bar Association emphasizes the importance of selecting your agent carefully. It's also wise to consider appointing a backup; however, it is best to pick only one person, to avoid conflicts over your care. Some states prohibit certain people from being your agent, most commonly doctors are not allowed. But you should certainly tell your doctors – and all of the people closest to you - your wishes, to increase your chances they'll be carried out.
