A major leadership shakeup followed a series of articles in Washington Post and subsequent media reports about substandard conditions and bureaucratic problems affecting the care of injured soldiers returning from Iraq and Afghanistan to Walter Reed Army Medical Center, in Washington, one of the military's highest-profile and busiest medical facilities, and its outpatient facilities.
The controversy started in February of 2007, but lawmakers have said reports of the failures go back several years.
